Physical Examination

Students registering for Athletics must have a current physical examination (in-person) uploaded before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and uploaded at the time of online registration or before online registration closes for the current season. Please be sure the form uploaded contains 1) statement of clearance for sports participation 2) date of the exam and 3) a physician's signature.

Physicals are valid for 1 year from the date of the last exam.
